For Atrium Health Floyd Teammates

To be eligible for benefits, you must be a regular full-time or part-time teammate of Atrium Health Floyd. Some plans have additional requirements, such as being scheduled to work at least 20 hours a week to be eligible for Medical, Dental, and Vision coverage.

You may enroll eligible family members for Medical, Dental, Vision, and Dependent Life coverage. Dependent coverage is also available under some supplemental plans.

Dependents

Eligible family members may include: 

  • Your legal spouse 
  • Your children up to age 26, regardless of their student or marital status or residency 
  • Your children of any age who are mentally or physically disabled and depend on you for support. (Note: You must certify the disability within 31 days of the dependent’s 26th birthday independently with the medical, dental, and vision providers. You will be asked periodically to resubmit proof.) 
    • Eligible children are defined as natural children, legally adopted children, stepchildren, and children for whom you have legal guardianship. See each summary plan description for the specific definition of an eligible dependent or if you have questions about your benefits. 
    • If you are adding a dependent, you must upload the appropriate documentation (birth certificate, adoption papers, marriage certificate, etc.) into CORE Connect for dependent verification. You will also be required to upload the appropriate documentation with Alight Dependent Verification Services. Failure to do so will result in your dependents losing coverage. Federal regulations require that you also provide Social Security numbers for all of your covered dependents.

Eligibility: Working Spouse Rule

If an employee’s spouse can purchase group health coverage that meets ACA minimum requirements through an employer, the spouse is NOT eligible for coverage under Atrium Health Floyd medical plans. You may be asked to complete a working spouse affidavit to certify there is no access to other medical care coverage if you enroll a spouse.  

Making Changes to Your Benefits 

You can make changes to your benefit elections upon new eligibility, when you have a qualifying event or during Annual Enrollment. See more information on the Teammate Benefits Enrollment page.

Please, note that ID cards for benefits with Day 1 coverage will be issued, if applicable, after the benefits enrollment is completed with the appropriate carrier. See more information on the Teammate Benefits Enrollment page.

1New hires are eligible to enroll and set up to contribute elected percentage as soon as administratively possible. Eligible new hires and rehires will begin to receive the matching contribution in the first pay period following 90 days of employment.

2PTO is earned from the first day of employment and may be scheduled after satisfactorily completing the 90-day observation period.

  • Floyd matches 100% on the first 3% of the employee’s contribution 
  • Floyd matches 50% on the next two percent of the employee’s contribution 
  • The maximum matching contribution from Floyd is four percent. To earn this maximum match, you must contribute 5%. 
  • The 401(k) savings plan and the 401(k) Roth savings plan are subject to a combined Federal limit and Floyd matching contribution.

Temporary teammates and Contract Workers are not eligible for benefits.
